Evolus Rewards™️ utilizes the patient’s phone number as the primary method of identification. The phone number is required to complete enrollment and redeem available rewards. Without the patient’s consent for their phone number to be contacted by Evolus, they will be unable to participate in the Evolus Rewards™️ program.
