How does my account opt in to Evolus Rewards™️?

Accounts can opt- in to Evolus Rewards™️ via the Evolus Practice App or My Evolus Portal at MyEvolus.Evolus.com.

Once logged in, select the Rewards tab, and click on “Opt-in”.

Please note you will need to have purchased a minimum of ten (10) vials in the current or previous quarter to be eligible to opt into Evolus Rewards™️.

How can I opt my practice out of Evolus Rewards™️?

Anyone with Admin access can opt your account out of Evolus Rewards™️ by visiting the Account Management function within the Evolus Practice App or My Evolus Portal.

Is my practice able to use Evolus Rewards™️ without purchasing Jeuveau® vials?

An account will be temporarily marked inactive and unable to use the Rewards program if they have not purchased Jeuveau® (prabotulinumtoxinA-xvfs) vials in the previous 12 months. An order of a minimum of 10 (ten) vials is required to re-activate the account.
