Evolus Rewards™️ is our modern patient rewards program that provides patients with an instant $40 off Jeuveau® (prabotulinumtoxinA-xvfs) treatment every 90 days.

What do I need to sign up for Evolus Rewards™️ as a patient?

All you’ll need to sign up for Evolus Rewards™️ is your mobile phone number. This will act as your account number and is all you’ll ever need to access your account and receive discounts with Evolus Rewards™️. When enrolling, we’ll ask you to provide additional details to complete your account profile including your name, email address, zip code, birth month/year and aesthetic treatment history.

If you have any difficulty enrolling via text message, you can elect to enroll instead by email. You will still need an eligible phone number to enroll via email as the phone number will be required to receive discounts.

How will I receive my $40 reward?

When checking in for a Jeuveau® (prabotulinumtoxinA-xvfs) treatment using Evolus Rewards™️ at your local participating practice, you will need to provide a magic phrase (sent to your mobile phone via text message or phone call) to validate the redemption of an available reward. Your practice (if they participate in the program) will then offer you $40 off directly from the total of your Jeuveau® (prabotulinumtoxinA-xvfs) treatment.

If there are changes to the program structure, will this impact an existing, unused reward that I already have?

Evolus Rewards terms are subject to change at any time. In advance of any change that will impact existing, unused rewards, all members will be notified.
