A patient can either elect to enroll themselves or be aided through enrollment by a staff member at a practice. In the situation where practice staff are facilitating the patient enrollment, this can be performed when checking patients in on app.evolus.com. The patient will need to provide their mobile phone number, and if this number is not assigned to an existing Evolus Rewards patient profile, it will prompt enrollment. The patient will need to provide consent to receive communications from Evolus via their phone number after which time they will receive a text with a link to complete their registration.
If the patient is unable to receive the enrollment link via text message then you can elect to send them this via email instead.
When the enrollment process has been completed the patient will receive their first reward via text message or email (depending which communication channel they completed enrollment).
